Clergy Excellence DCOM Forms

If you are a dCOM Chair, Secretary, or Registrar, we have updated the report forms and process as of July 1, 2023.

Previously, three forms were used for dCOMs: the Action Report, the Candidates Narrative, and the General Meeting Minutes form. These forms have been combined into one with controls to show the necessary questions depending on the kind of reporting you are doing. These links to these forms have been updated below.

In addition, dCOM Secretaries who record the minutes are to send their minutes to the dCOM Registrar who can complete the action report and insert the minutes for a one time upload into the candidates file on Basecamp.
