Annual Reporting for the Deacon and Provisional Deacon

As a deacon or provisional deacon in the Virginia Conference – appointed to a local church or beyond the local church – the Discipline requires annual reporting on the official report form to your local church charge conference and to the Board of Ordained Ministry.

All Annual Report forms are due to the Center for Clergy Excellence by January 15. The Center will forward your report to the Board of Ordained Ministry.

The Virginia Conference has moved to digital submission of all charge conference report forms, automatically filling in contact information you have previously entered and sending the report to all who should receive a copy. Go to www.evc.vaumc.org to login or for assistance with this program, send an email to [email protected].

If this way of submitting does not work for you, you may request a paper copy of the form from the Center for [email protected]. Remember to mail a copy of this form to all those persons or agencies listed at the bottom of the form.

For deacons appointed to a local church – either as a member of the Virginia Conference or as a member of another conference appointed in the Virginia Conference – please be attentive to the Appointment Review Process and the appointment request form specifically for deacons and provisional deacons. The deadline for submitting this form is January 31 so that the Cabinet may consider your appointment for the next conference year.
