Campus Ministry Forms

Campus Ministers must submit several yearly ministry reports to the Virginia United Methodist Conference’s Board of Higher Education and Campus Ministry and their District Superintendents. Each report can be accessed by clicking on the forms below. They need to be completed and filed according to each report’s instructions. REPORTING FORMS

Beginning in 2025, ministries may submit reports electronically. In the 2025 reporting cycle, campus ministers and ministries may submit either the new electronic forms or the traditional individual reports linked below.

Online Reports

Campus Ministries using the updated system will submit the following reports according to the following schedule.

Electronic Campus Ministry Report Schedule

– Personnel Committee Evaluation: 2025 Evaluation of Wesley Foundation Director/Campus Minister by the Local Board Personnel Committee Due June 1

– Director’s Report: 2025 Evaluation of Campus Ministry by the Director Due June 1

–  For ministries with conference-owned property, a Property Report: 2025 Campus Ministry Property Update Due June 1

–For UM Clergy  Preference Forms Submitted to the District Superintendent – Due December 1

– Form for Campus Ministers or Chaplains  (Word) (Indicates personal preference for reappointment)
– Form for Campus Ministers or Chaplains (pdf) (Indicates personal preference for reappointment)
– Form for Local Board or College/University President (Word) (Indicates board preferences for reappointment)

Annual Financial Reports Transmittal Form (Requests a copy of the annual budget and income and expense report from the previous year)
Annual Financial Review Form – Submit Not Later than 1 June for previous fiscal year.
